The game centres around a disturbed killer known only as The Antagonist, whose stated aim is to kill as many people as he can.

The main objective of the game is to attack human-like characters; civilians and policemen in varying environments using an arsenal of weapons including a shotgun, flamethrower, knife and grenades. The police appear regularly in an attempt to take-down the killer, occasionally succeeding. All violence is realistic and accompanied by a dark red blood effect. If a target is wounded on the ground, the player may trigger an ‘execution’ feature, which shows the killer approach the injured victim and execute them in graphic ways. These include shooting victims point-blank in the face, stabbing them in the neck, head or eye, and stomping on their head which causes their skull to shatter to pieces. The game also features frequent use of the words ‘fuck’ and ‘fucking’.
